FORM INSTRUCTIONS:

  • Complete this form in your browser, then press the ‘Submit‘ button at the bottom of the page.
    • After clicking ‘Submit’ you will be given links to print your completed form and the accompanying Payment Information Form.
    • If you would prefer to print a blank Restoration Order Form to complete by hand, please click here for a printable copy.
  • For extra security, please complete your Payment Information Form by hand after printing.
    • Corvette Specialties of MD West does not charge any upfront fees, but we do require a card on file before we begin services.
  • Both forms should be included in your shipping box with your parts for restoration.
  • Be sure to check out our Packing & Shipping Tips to give your parts the best chance of reaching us safely!
